1. Submit completed form not later than Friday, 7 May 2021.

2. Submit the following documents along with the application forms.

One (1) Certified copy of each certificate, diploma and degree

One (1) Certified copy of Birth and Marriage Certificate

One (1) passport photograph

Copy of application receipt

Medical certificate verifying applicant’s ability to pursue the programme.

3. Application Fee BDS $50.00 - non refundable.

4. Application forms not properly completed or not accompanied by relevant documents or application receipt will NOT be considered.

5. Please arrange for transcripts from Colleges or Universities to be sent to Erdiston College in time for the application deadline.
