A valid Police Certificate of Character.

You need to apply for the Police Certificate of Character at least three (3) days before completing this form.

You must have at least four CXC certificates. Three of which must be English, Mathematics and a Science subject at Grades I, II or III.

Photo Requirements

Your passport size photo must be:

In color
Photo must be 2 inches by 2 inches

The height of the head (top of hair to bottom of chin) should measure 1 inch to 1 3/8 inches (25 mm - 35 mm)

Make sure the eye height is between 1 1/8 inches to 1 3/8 inches (28 mm – 35 mm) from the bottom of the photo. See template below:

Taken in clothing that you normally wear daily.
Uniforms should not be worn in your photo, except religious clothing that is worn daily.

AUTHORITY AND DECLARATION

I declare that the information in this declaration, and any other documents completed by me in support of my application for employment in the Barbados Fire Service and the information provided by me during any assessments and interviews in connection with my application for employment in the Barbados Fire Service, is true and correct in every detail.

I also understand that any offer of employment to me in the Barbados Fire Service will be expressly on the basis that the information that I have provided in relation to my application for employment is true and correct in every detail.

I understand that any incorrect statement in connection with my application for employment in the Barbados Fire Service may lead to a rejection of my application for employment, or, if I am offered employment in the Barbados Fire Service, make me liable to disciplinary action, which may include termination of employment.

Furthermore, I hereby authorize the Barbados Fire Service to conduct further checks upon submission of my application for employment, in respect of charges and offenses, including disclosure of further information in relation to any associated police history information.

I hereby consent to the ‘Authorised Agent’ providing the Barbados Fire Service information relating to any convictions that may be laid against me upon submitting an application for employment. In particular, the Authorised Agent’ may disclose any personal information arising from such checks and convictions to the Barbados Fire Service.

If successful in my application for employment I commit to:

Giving 100% into training and learning as a Fire Officer, including assessment requirements.

Undertaking on a full-time basis, a total of 6 months of training. This includes three (3) months of Firefighter 1 & 2 training and three (3) months of Emergency Medical Technician training.

Pass the two (2) year Probationary period.

Comply with the Fire Service Act Cap.163, the Public Service Act, Cap.29 and any other relevant legislation governing Public Officers.
